We have received your provider referral submission. Our care team will contact you via email within 24-48 hours. Once we receive your information, you’ll be invited to create an account in our secure electronic health record system, Osmind, where you’ll need to complete all required intake forms and surveys.

After your paperwork is successfully completed, we’ll reach out to schedule your complimentary consultation via Zoom. Please note that while consultations are offered at no cost, we require 24 hours’ notice for cancellations or rescheduling.  We maintain this policy to ensure all patients seeking innovative treatment options have access to timely care.
